Intense Pulsed Light (IPL) and laser treatments use concentrated light energy to target specific skin concerns, such as pigmentation, vascular lesions, hair reduction, and skin rejuvenation. Results vary depending on skin type, treatment area, and individual response.

Possible Risks & Side Effects. I understand that IPL/laser treatment may involve the following temporary side effects:

• Redness, swelling, and mild discomfort

• Heat sensation similar to sunburn

• Temporary pigmentation changes

• Bruising or blistering (rare)

• Crusting or scabbing (rare)

• Risk of burns if aftercare instructions are not followed

I acknowledge that while every effort is made to ensure safety, no guarantees can be offered regarding results.

Pre‑Treatment Requirements. I confirm that I have been informed of the following:

• Avoid sun exposure, tanning beds, and self‑tan for at least 2–4 weeks before treatment.

• Do not use retinoids, acids, or exfoliating products for 5–7 days prior.

• Shave the treatment area (for laser hair removal) 24 hours before the appointment.

• Arrive with clean skin, free from makeup, deodorant, or lotions.

Aftercare Instructions. I understand that after treatment I must:

• Avoid heat (hot showers, saunas, strenuous exercise) for 24–48 hours.

• Avoid sun exposure and apply SPF 30+ daily.

• Do not pick or scratch the treated area.

• Avoid retinoids, acids, and exfoliation for 5–7 days.

• Follow the clinic’s instructions for any discomfort or reactions.

Consent & Acknowledgement

By signing this form, I confirm that:

• I have been fully informed about the treatment, risks, benefits, and alternatives.

• I have had the opportunity to ask questions and all questions have been answered.

• I understand that results vary and multiple sessions may be required.

可能的风险与副作用. 本人理解IPL/激光治疗可能涉及以下暂时性副作用：

• 结痂或结疤（罕见）

• 若未遵循术后护理指导，存在灼伤风险

本人知悉，尽管治疗方将全力确保安全，但无法对治疗效果提供任何保证。

治疗前要求. 本人确认已获知以下须知事项：

• 治疗前至少2-4周内避免日晒、使用美黑灯床或美黑产品

• 治疗前5-7天停止使用视黄醇、酸类及去角质产品

• （激光脱毛前）需在预约治疗前24小时剃除治疗区域毛发

• 到诊时需保持皮肤清洁，无化妆品、止汗剂或润肤露

术后护理指导. 本人理解治疗后必须：

• 24-48小时内避免热刺激（热水淋浴、桑拿、剧烈运动）

• 避免日晒并每日涂抹SPF30+防晒霜

• 不得搔抓或剥脱治疗区域

• 5-7天内避免使用视黄醇、酸类及去角质产品

• 出现任何不适反应时遵循诊所指导

知情同意声明通过签署本文件，本人确认：

• 已全面了解治疗方案、风险、获益及替代方案

• 已获得充分提问机会且所有疑问均获解答

• 理解治疗效果存在个体差异且可能需要多次治疗
